*CONSENT TO THE COLLECTION, PROCESSING AND TRANSFER OF MY PERSONAL DATA:

I consent to the collection, processing and transfer of my personal data in accordance with Article 6(1)(a,c,e and f) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, Official Journal of the EU L 2016, No. 119, p.1 as amended) in connection with the Act of 20 July 2018, Law on Higher Education and Science (Journal of Laws of 2018, item 1668 as amended); and executive acts issued on its basis, as well as internal acts adopted at the University - transferred to the Powiśle School of Higher Education - Organizer of the International Scientific Conference and the 2nd International Scientific Conference entitled. "The health care sector in the face of the SARS coronavirus pandemic - CoV- 2" in connection with my participation in the Conference and the use of my image, submitted materials and statements made by the Organizer for the purposes of information, promotion and publication of photographs and my image in the mass media. This consent is indefinite. I am aware that granting this consent is voluntary, and lack of it will result in inability to participate in the conference.

** CONSENT FOR GRATUITOUS DISTRIBUTION:

I give my consent to the gratuitous dissemination of my name, image (photos), interviews and recordings with my person related to the Conference in which I participate in the information and promotional materials of the Powislańska Szkola Wyższa and I will not make any claims on this account. Consent is not limited in time or territory and covers all forms of publication.

*** MARKETING CONSENT:

I consent to the processing of my personal data in order to receive by electronic means from the Powislańska Szkoła Wyższa at the electronic address indicated in the application form for the Conference, in accordance with the Act of 18 July 2002 on providing services by electronic means (Journal of Laws of 2002, item 1204, as amended) commercial and marketing information, i.e. to inform me, among others, about: the training offer of the University, events recommended by the University, competitions, trainings, offers, including services and offers of external entities. I am aware that my consent may be revoked at any time, which will result in removal of my data and e-mail address from the distribution list. .

1. The administrator of my personal data is Powiślańska Szkoła Wyższa with its registered office at ul. 11 Listopada 29, 82 - 500 Kwidzyn, entered under number 166 in the register of non-public universities, hereinafter referred to as: the University or PSW.

2. The Administrator of the personal data has appointed a Personal Data Protection Inspector supervising the correctness of the processing of personal data, who can be contacted by post at the correspondence address: Powiślańska Szkoła Wyższa, ul. 11 Listopada 29, 82 - 500 Kwidzyn or by sending an e-mail to: ido@psw.kwidzyn.edu.pl.

3. The legal basis for the processing of personal data is Article 6(1)(a), (c), (e) and (f) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, Official Journal of the EU L 2016, No. 119, p.1 as amended) in connection with the Act of 20 July 2018, Law on Higher Education and Science (Dz.U. of 2018, item 1668 as amended); and executive acts issued on its basis, as well as internal acts adopted at the University.

4. My personal data may be transferred to the following entities:

a. persons authorized by the Administrator to process my personal data in the performance of their official duties;

b. entities to whom the Administrator subcontracts the performance of activities which require the processing of my personal data (so-called processors), i.e. companies providing and operating selected IT systems and solutions, postal and courier service providers, etc;

c. subcontractors in connection with commissioned by the University activities carried out on behalf of the University to organize the Conference;

d. entities providing legal assistance and legal representation on behalf of the University;

e. entities dealing with IT support of the data controller;

f. entities or bodies authorized by law, including for the purposes of reporting and performance by the Controller of PSW statutory, regulatory, information, archiving and statistical obligations resulting from the Act of 20 July 2018, Law on Higher Education and Science (Journal of Laws of 2018, item 1668 as amended); and executive acts issued on its basis, as well as internal acts adopted at the University.

5. I have the right to:

a. Access to the content of my personal data;

b. rectify my personal data;

c. limit the processing;

d. have my data erased;

e. to object;

f. withdraw consent to the processing of my personal data at any time without affecting the lawfulness of such processing;

g. lodge a complaint to the supervisory authority, which in Poland is the President of the Office for Personal Data Protection, ul. Stawki 2, 00-193 Warsaw.
